[Washoe County] REFUND CLAIM FORM -with W9

This electronic claim form is available for Incline Village and Crystal Bay property owners who have verified that a parcel they owned between July 1, 2003 and June 30, 2006 is on the list of parcels that may qualify for a refund.

When filling out this form, you will have the opportunity to complete the W9 with the Claim Form. When you finish filling out the forms and are ready to submit, you will also have the opportunity to attach additional documentation with your Claim Form. A Consent to Claim Refund is required to be submitted with your Claim Form if parcel ownership includes multiple co-owners.

Refunds will begin on July 1, 2021 and are expected to be completed by June 30, 2024.
